MOD MESSAGE/ANNOUNCEMENT Community feedback needed (please vote)

Hi all,

Our mod team was recently approached by BetterHelp, an online subscription-based therapy provider, to do an AMA within our subreddit by a licensed therapist that specializes in child loss, perinatal mental health, and anxiety. They are willing to provide credentials to our mod team to verify the individual responding to questions, but our team has reached a moral impasse that we need the community to weigh in on.

Up until now, our team has had a blanket rule to not allow any companies, researchers, institutions, or anyone else with private interests to post within our community because our space is filled with so many vulnerable humans going through extremely difficult situations. The fertility space, in general, is ripe with opportunity for those that might take advantage of this vulnerability, much less a space like ours. That said, the message they sent was clearly written very thoughtfully, and the provider they’re willing to assign to the AMA would be willing to answer people’s questions about their worries, maybe provide healthy coping mechanisms, and describe the subspecialties of therapy that could be most beneficial to the people here.

IF we allow this AMA, it needs to be with the majority support of our community, so we are opening a poll for feedback. Other than voting in the poll, upvoting this post and providing even a short comment with your thoughts/concerns/ideas would help boost the visibility of this announcement to the maximum amount of sub members.

Of course, the major issue for us as a mod team is that we are NOT endorsing BetterHelp as a specific company or resource. For folks that are in areas where they aren’t able to find an in person provider, online therapy through any number of companies may be helpful. We are not partnering with them and the language in the AMA will be very clear about that. We will also be making sure that the provider answering questions in the AMA does not promote the BetterHelp brand.

If you’ve read this long, thanks for bearing with us. We appreciate how the people of this community come together to support one another and are grateful to help moderate.

Would you be interested in and supportive of an AMA provided by a therapist from BetterHelp, with the limitations outlined above?

Ultrasound FAQ Reference Guide

As an OBGYN ultrasound tech working at the general level here's a few things I would love the community to know based on common questions I see.

Most important rule of thumb: Worry when your doctor tells you to.

OBGYN docs are going to pay the most attention to the fetal pole and heart rate. Once a fetal pole is seen, the CRL (crown rump length) measurements are gold standard.

I'm worried about the heart rate:
-Check out this resource for pregnancies dating less than 7 weeks. Make sure to refer not to your LMP dates, but measured CRL dates: https://radiopaedia.org/articles/fetal-bradycardia?lang=us
-After 7 weeks a normal heartrate is between 120-180. It can temporarily dip below or temporarily rise above this range, but prolonged time above/below will definitely get your doctor's attention especially in third trimester. Generally out of range heart rates trigger NST (non stress test) or more monitoring.
The measurements are off:
Baby measures behind LMP. This is actually pretty common, we often see dates off up to a week either direction. We don't ovulate on the exact same day of the cycle each month or even have the same length of cycle. With irregular periods, PCOS, or unknown dates even larger gaps can be perfectly fine. If you aren't sure of your dates, trust the ultrasound. Even if you are sure of your dates, don't panic if they are a week off. Ultrasound measurements in early pregnancy are +/- 5 days.
I'm in limbo and don't know if my ultrasounds are diagnostic for loss:
By ultrasound there are strict parameters to diagnose loss. No one wants to get this wrong. Check out this resource for a list of parameters diagnostic for loss: https://radiopaedia.org/articles/early-pregnancy-loss-1?lang=us
Note, there is a second list of "suspicious for loss". Keep in mind the second list is not diagnostic, things can still turn out just fine.
I don't know when I should see what on ultrasound:
Things can vary a bit from person to person and do change *quickly* in early pregnancy. Check out this resource for what you can generally expect to see:
https://radiopaedia.org/articles/early-pregnancy?lang=us
Should I go to a boutique or get a private scan? Or push for an earlier scan?
They aren't legal where I am but here's a few things to consider if you plan to book a private scan. First, what are the qualifications of the person running the ultrasound? An ultrasound is only as good as the one performing it. It sounds as if these boutiques are NOT permitted to give much/any information. Will you spiral if you believe you see bad news and they cannot confirm or deny it? Also it seems that many cannot do vaginal ultrasound, which will make seeing a baby under 7 or 8 weeks challenging. If I had the option of a boutique ultrasound I would personally wait until 8 weeks and I would book it for a weekday. If they see something bad, it is not ideal to find out over a weekend when there is no option to follow up with your doc. An earlier scan often causes more anxiety, not less. It's SO hard to wait. But if you can tolerate it wait until 8 weeks or so.
The gestation sac is small/big:
-Gestation sac measurements are not terrifically accurate. Where I work once a baby is seen the gestation sac isn't measured. A mismatch between baby's size and the gestation sac size isn't uncommon. That said, a mean sac diameter of 25 mm or more without a baby is diagnostic for loss.
The yolk sac is small/big:
It starts teeny tiny, and will get a bit bigger. Once a baby with a good heartbeat is visualized, the docs I work for don't seem to care about the size of the yolk sac. A failed pregnancy with an expanded yolk sac (more than 6 mm) is a suggestive of chromosomal abnormalities.
We don't know where the pregnancy is:
The statistically most likely outcome is either a chemical pregnancy or just coming in for ultrasound too early for visualization. Ectopic can't be ruled out, and many wise ladies in the this sub have unfortunately experienced this and can give more info than I can. Statistically it's believed that only 1-2% of pregnancies are ectopic, but listen to your body. If the location is unknown, you should be getting close monitoring and follow up until the location is known or the pregnancy has ended.
-For a resource on ectopic pregnancy: https://radiopaedia.org/articles/ectopic-pregnancy?lang=us A note, a pseudosac is visualized in only 10-20% of ectopics. Therefore a sac in the uterus is suggestive of intrauterine pregnancy but can't be proven until a yolk sac is also present.
What about my HCG numbers?
I'm not an expert on this at all. But here's a resource: https://radiopaedia.org/articles/beta-hcg-1?lang=us
Why didn't my ultrasound tech tell me anything? And/or why were they so optimistic? Or, I had an ultrasound in the ER and they were very pessimistic.
Many locations an ultrasound tech can very much lose their job if they say aNyThInG. Where they are permitted to say certain things some ultrasound techs may just be hoping the best. We work with the public and we have seen some wild stuff. Additionally no one wants to call it a loss unless its 1000% certain. We know how devastating that news is. A side note here, often an ultrasound in the ER is not an ideal experience for the patient. The ER is staffed with techs that are much more versed in gallbladder and liver scans than they they are early pregnancy ultrasound. Don't hesitate to go the ER if you are having concerning symptoms, but do know the ER doc will make sure you aren't actively dying then will likely refer you back to your OB for follow up.
I'm just so anxious about it all:
Someone awesome wrote out these mantras. Maybe they will help: https://www.reddit.com/r/PregnancyAfterLoss/comments/14uhbdg/commandments_of_pregnancy_after_loss_this_is_now/
Also a podcast I personally found very helpful with anxiety: https://www.oneyoufeed.net/russ-harris-part-1/
https://www.oneyoufeed.net/russ-harris-part-2/

Today you are pregnant and this baby is so loved. I really am hoping the best for each of you. Those of us who have experienced loss.... you didn't do anything wrong, I promise. This is a club no one deserved to be in. And you are not silly, foolish, or dramatic for grieving your little babe for as long as you need to.

This post is written in honor of Eli, who died long before I could meet him. Rest in peace, buddy. I miss you tremendously ❤️

When to tell older siblings I’m pregnant

• Upvotes

I am currently 9w4d after a euploid embryo transfer. We have had weekly scans since six weeks and everything has looked good. We have our last scan with our fertility clinic on Thursday at 9w6d. We got our NIPT back today and it was negative. My husband and I plan to look at the gender together tomorrow. Then I planned to tell my daughter (4) and my son (18mo so he won’t care lol) on Friday when I am 10 weeks and after we had graduated from the clinic. The closer it gets though the more paranoid I feel (we just came off a chemical pregnancy) and then I think maybe it’s too soon? I’m getting to the point where I would like to share with immediate family too, but I want my daughter to be the first to know. When did you tell your kiddos/what would you do?

Advice Needed High hCG surge (7,200 at 5w1d) but ultrasound only shows a small, empty gestational sac. In limbo and terrified of a blighted ovum. Has anyone experienced this?

I am completely spiraling right now and desperately need some insight or reassurance. I am currently in ultrasound limbo and terrified that I am facing a blighted ovum, but my hCG numbers are throwing everyone off.
Here is my exact timeline:

Oct 4th/5th: Positive ovulation strip (tracking closely).
Oct 14th: Faint positive pregnancy test.
Oct 18th (approx 4w0d): Went to the ER due to weird cramping on my left side because I wanted to rule out an ectopic. HCG was 177. Ultrasound showed nothing (too early).
Oct 20th: HCG rose to 445.
Oct 23rd (approx 4w5d): HCG jumped to 2,500. Ultrasound showed a gestational sac, but the tech said it was "too small to confirm it's a pregnancy."
Oct 26th (yesterday, 5w1d): HCG surged to 7,200. Gestational sac was seen again on the pelvic ultrasound, but it still looked empty. The radiologist didn't say much, but the medical team admitted they are concerned because they aren't seeing a yolk sac or an embryo yet at this hCG level.
They have scheduled me for YET ANOTHER ultrasound tomorrow (I'll be 5w3d) in the fertility department to use their high-definition equipment.
I am so, so worried. Why is my hCG surging so beautifully into the thousands if there is no embryo or yolk sac visible yet? If it's a blighted ovum, can hCG really jump from 2,500 to 7,200 in just three days while the sac stays tiny and empty? I don't have a family history of twins and didn't use fertility treatments, so I don't think it's a heterotopic or multiple pregnancy.

Has anyone ever had numbers this high with an empty sac at 5 weeks and gone on to see a baby a few days later? Or is this a classic sign of an early loss/blighted ovum? Please be honest, the waiting is killing me.

Advice Needed Title: What's been the hardest part about exercising during pregnancy?

For those of you trying to stay active during pregnancy, I'm curious what the biggest challenge has been.

Is it knowing which exercises are safe, finding the energy, staying consistent, dealing with changes in your body, or something else entirely?

I feel like people talk a lot about the benefits of staying active, but not always about how hard it can be to actually make it happen.

What's been the most frustrating part for you?

18mm gestational sac with yolk sac but no fetal pole at 5w4d — anyone have a similar experience?

Hi everyone! I’m hoping to hear from anyone who has experienced something similar, especially those who went on to have a healthy pregnancy.
I had a transvaginal ultrasound today (October 27), and they found a single intrauterine gestational sac measuring **18 mm (approximately 6w5d–6w6d)** with a visible yolk sac, but **no fetal pole or embryo was seen**.
A little background:
**LMP:** September 12
Gestational age based on LMP:** 6w3d
**Confirmed ovulation:** October 2
**Gestational age based on ovulation:** 5w4d
**Gestational sac:** 18 mm
**Yolk sac:** Present
**Fetal pole:** Not visible
**Sac shape:** Normal
**HCG:** 127 on October 13 and 645 on October 16
My doctor said it could simply be too early and recommended another ultrasound on November 6.
What concerns me is that my gestational sac is measuring closer to 6w6d, even though I know I ovulated on October 2, which would put me at only 5w4d. I’m confused about why the sac would measure so far ahead without a visible embryo.
I also experienced a missed miscarriage earlier this year, so I’m understandably anxious about this ultrasound.
**Has anyone had an 18 mm (or similarly sized) gestational sac with a yolk sac but no fetal pole, and then gone back a week or so later to see a healthy embryo with a heartbeat?**
I’d especially love to hear from anyone who was certain of their ovulation date and had a gestational sac measuring ahead of their actual gestational age.
I’m trying to stay hopeful while also being realistic. I know every pregnancy is different, but hearing others’ experiences would mean a lot. ❤️

Low Heart Rate at 6+3

This is an IVF pregnancy with donor eggs. There’s only one more embryo left after this one. I started bleeding off and on at 6 weeks and went in for my first ultrasound at 6 weeks 3 days. They found a SCH, which explains the bleeding (which has now stopped for over 24 hours). However, the baby is measuring slightly small at 3mm and the ultrasound tech had a very hard time getting the heartbeat. She would see it and try to record for like half a second, and then it would disappear from the screen. She recorded both 83 and 69bpm after about half an hour of trying, which are both way too low. We are coming back in a week which should give us more clarity but are preparing for a loss. Everything about this past year has been so incredibly hard. I was so shocked when I tested positive because nothing has really worked out for us in this whole process. Any words of wisdom, consolation, or dare I ask, hope?

Advice Needed Beta hCG 40.2 at 11-13 DPO - any success stories or advice?

• Upvotes

We were working with a fertility clinic and had just completed our initial bloodwork when we unexpectedly conceived naturally.

My first beta on 10/27 was 40.2, and I’m worried it’s low. I had a possible faint line 10/24 and a definite positive 10/25. Ovulation was likely 10/15–10/16, and my next beta is Friday 10/30.

Has anyone started around 40 at 11–12 DPO and gone on to have a healthy pregnancy? Also, what were your next steps after a positive with a fertility clinic? When did you have your first ultrasound?

I’m feeling anxious and would really appreciate hearing others’ experiences!

6w Back Pain

TW: miscarriage

I’m currently 6w4d pregnant and I am beyond anxious about every single symptom I’m feeling (or not feeling) after having two back to back miscarriages. I started feeling back pain today and the last time I felt this, I bled shortly after and lost the baby. Is back pain a normal pregnancy symptom or a sign I could be miscarrying?

I have a scan tomorrow but I’m just too worried that I’m going to see another baby with no heart beat.

Sad 149hr hCG doubling time, there’s no hope, right?

DPO unknown, but estimated around 20-25th of September.
First faint positive on Oct 3rd. Initial slow line progression (and a MMC earlier this year) made me extremely anxious, so I convinced my gp to do a blood draw.
Oct 8th hCG was 230.
Oct 12th hCG was 630, which was within the normal range so my gp assumed I was just earlier than expected.

Anxiety didn’t go away, I had mild pregnancy symptoms and just didn’t have a good feeling about it, so I got sent for an ultrasound on October 22nd. The gynaecologist was clearly in a big rush but transvaginal ultrasound showed “nothing” and likely because it was simply too early according to her (and difficult to scan due to high BMI).
They did another blood draw that day, which came back at 5830. That made me slightly relieved, it still seems to be within the normal range? But the US was then not at all promising since nothing was seen at that hCG level. I’m so confused.

Last blood draw was yesterday on October 26th and I got the results today: 9090. Which is a way lower increase than any success story I’ve been able to find.

I’m confused and worried, I couldn’t get a hold of the clinic today because the result came after their closing time.

I think I know this is the end, but my worry is if it’s ectopic.
I don’t have any symptoms of an ectopic pregnancy, no pains, bleeding or cramps. The gynecologist said there was nothing on the scan to indicate an ectopic, but I’m still terrified.
I’m scheduled in for another ultrasound on Thursday but I’m going to call first thing tomorrow to see if they’ll fit me in same day.

Advice Needed Clearblue weeks indicator, symptoms, anxiety.

I know what advice I would give to someone else in this situation, I would say stop testing, those tests are innacurate etc but it's so hard to take my own advice, I'm sure you understand 😅

I had a faint positive pregnancy test back on 9th Oct, I used up the frer's that I had and saw fantastic line progression, even though I had some very light spotting on and off for about 2weeks.. I also did a digital clearblue that said 1-2. 16th October I had beta done which was 215. I did another digital that said 2-3. Just over a week from this I decided to get another one, expecting to see 3+, but it still said 2-3. I know I should stop testing. I just have so much anxiety that something is wrong. For no good reason. The spotting has stopped. I have mild on and off nausea. But otherwise I feel completely normal.

This is baby number 5 for me but mine and my partners first together. My kids are 9 and up, I have just turned 35. I think my age now is worrying me a bit?

I have my first scan in exactly 48 hours from now. 😬 I ran out of ovulation tests this cycle, so never saw a positive, but I believe my first positive I was approx 8-10 dpo ..

Progesterone Dip

I’ve had 3 blood draws. At the first one I was 2 weeks and 3 days. my HCG was 9 and my progesterone was 22. 3 days later I had another draw, my HCG was 241 and my progesterone was 35. Yesterday, at 4 weeks and 2 days I had my third draw. When I got the results back my HCG was 3,759 but my progesterone had dropped down to 18. I know it can fluctuate but that seems like such a large dip to me? My PCP has been doing the draws because OB won’t see my until I’m 8 weeks. She said this is totally normal and that she’s not concerned, and won’t put me on suppositories because too much could be bad for baby. Has anyone else had a similar experience? By the time she called me the OB I plan on using was closed so I can’t call them until tomorrow. But this is my first baby and I’m downright terrified. All the women in my family needed the suppositories to carry to term and at this point I’ve had so many women tell me their miscarriage horror stories that I’m convinced now something is going to happen to my baby. And I just want it to be okay.

has anyone had something similar happen?

Advice Needed HCG levels 6w3d

My HCG levels were 57,622 at 6w3d. I had light spotting today which triggered the test. They also set up a scan on Thursday which I’m terrified for given a prior MMC in July where pregnancy ended at 6w2d.

I know this HCG is on the higher side. Does that provide any indication that the pregnancy is likely viable? I know ultrasound is the real answer but trying to work with the data I have today. Thank you so much…

Advice Needed Healthy pregnancy with polyp? TW Loss

So today I have my first SIS after reoccurring losses and they found a small polyp. I have a follow up with the fertility specialist tomorrow and I’m making my list of questions.

Background: I have had 3 pregnancies in the last 9 months and all were intrauterine and ended around 6 weeks. My level for the blood clotting disorder came back slightly elevated so the plan for next pregnancy was blood thinner and progesterone.

From what I’m seeing, polyps are more of a concern for implantation-which I haven’t had an issue with.

Has anyone had a healthy pregnancy with a small polyp?

Has anyone’s doctor said their miscarriage was likely due to a polyp even with successful implantation?

I have had 3 surgeries this year and really would like to avoid it if it not completely necessary.

TW/TMI Cramps/spotting/low start Hcg and SUPER gassy

Hi all

Will try and keep this brief… looking for some reassurance and positive outcomes with similar stories. I have history of recurring miscarriages between 5-6 weeks. Currently 18 DPO- 4wk+5 based on LMP.

Just had a sonohysterogram this cycle and confirmed uterus and tubes we normal/open. Ended up getting a positive same cycle!

Timeline:

Dpo is estimated- not confirmed*

10 dpo (CD26)- first faint positive frer test

13 dpo (CD 29)- first Hcg 29.9

17 dpo (CD 33)- second Hcg 161.3

I have had tremendous gas and constipation since testing positive… last night, which was 17DPO, I woke up to intense cramping, not one side or the other, and passed a ton of gas before cramping went away, last about 20mins… when I went pee I noticed I was spotting. Brown spotting, haven’t filled a pad at all, only left two spots on a pad ever since but still on and off brown when wiping.

I haven’t had any other cramping since, but still passing a ton of gas.

I know I started with low Hcg, but doubling seems good and optimistic so far. But the spotting is freaking me out a bit.

Next Hcg beta is tomorrow/19 dpo.

Any similar stories would be awesome!

Blighted ovum?

So when I was 6 weeks I had light spotting and dr sent me to do HCG bloodwork. 1st one came out at 10893 at 6w3d and 2nd was 13201 at 6w5d. The nurse called me and said they expected my numbers to double so they sent me for a transvaginal ultrasound. I had the ultrasound at 7w2d and they saw a gestational sac but no embryo. They want to do another one next week to make sure but she said it might be a blighted ovum. Looking for other ppl that went through similar experiences :(

Advice Needed Very high hcg at 5w2d (22dpo)

Just got my latest hCG results back: 13,656 at 22 DPO (5w2d), which seems reallllly high, right?

My previous results were:
15 DPO: 1,008
18 DPO: 2,268

I know hCG levels vary a lot from person to person, but mine seem unusually high, even around twice the reported median for twin pregnancies at this stage.

I know high hCG doesn’t necessarily mean twins and that only an ultrasound can confirm anything. I actually have my first ultrasound scheduled for this coming Monday!

In the meantime, I’m really curious whether anyone else had similarly high hCG levels this early on and what the outcomes were!

Nervous and need advice!

TW: Pregnancy after loss

I had a chemical pregnancy a few months ago, and I’m now in my 8th week. I’m finding myself so scared of everything and honestly don’t know how to get through each day without worrying.

Last week, I was incredibly nauseous. It was miserable, but at the same time, it was reassuring in a strange way. This week, I’m feeling better and am nowhere near as nauseous. I know I should be grateful to feel some relief, but instead, I can’t stop panicking that something might be wrong.

I feel silly even considering calling my doctor or asking to move up my next appointment just because my symptoms have changed, but I also feel so helpless. It’s hard feeling like all I can do is wait and hope that everything is okay.
I even took a pregnancy test today, and the line was lighter than I expected, which obviously did nothing to ease my anxiety. I know testing at this point probably isn’t the best idea, so that one’s on me.

I guess I’m just struggling to navigate pregnancy after loss and trying to figure out how to get through the waiting without constantly fearing the worst. ❤️

Sad so anxious

so me (23) and my husband (22) have been trying for a year. I literally have an appointment Thursday with my OBGYN.

Well I got a positive pregnancy test 2 days ago. Here’s the problem.

I got a positive LH on 10/8. Oura says I ovulated on 10/10. My period was a couple days late- then came. Super light spotting for 2 days, then heavy bright red, then spotting for a couple more days. So I got my positive pregnancy test on (what was supposed to be) CD 6. Ive tested multiple times and with different brands and they are all positive. I also took a pregnancy test when I was late and in the middle of bleeding because I thought it was weird. Negative.

Soooo…the anxious part is that I got my HCG blood draw done because I was concerned and it came back at 15. I go back tomorrow for a redraw. So nervous this could be a CP :((

BFP QUEST hCG results better than expected?- TW Previous Miscarriage

I got my blood drawn last 10/22 (approximate 4 weeks, 10 dpo) and the hCG total quantitative came back as 45 mIU/mL got a bit concerned but also, I know I am testing super early.

Repeated the test again 4 days later 10/26 (4w4d, 14 dpo) and it came back to 411 mIU/mL !!

What is throwing me off is the Quest's hCG guideline table... I can't seem to make sense of it and it scared me to compare and see my numbers so low to what their standard reads.

Won't have my initial OB appointment until December 8th even though I mentioned repetitively that I had history of miscarriage and needed confirmation with an earlier appt. if possible. SO hard not to be anxious and stressed about it :(

Note: My cycles are an average of 32-33 Days. First day of LMP was 9/25 and OV on 10/12.

IVF pregnancy slowish beta rise 102 to 170

I am pregnant for the first time EVER through IVF. My partner and I have been trying for 2.5 years and have been through 2 egg retrievals and 5 transfers. I’d never gotten to a positive test until this time!

My first beta was 102.8 (10dpt) and 48 hrs later my second was 170.2 (12dpt). My clinic was looking for a 50% increase minimum so this meets the threshold, but they want me back in 2 days to make sure it’s continuing to rise appropriately.

I see a lot on here about people with much higher numbers and true doubles every 48 hours. Has anyone had similar #s and a viable pregnancy? Any info appreciated!
