Sorry this is still very WIP 😬

I read the same stories every time I come here.

"hi i got awful side effects" "I dont feel anything" "Hi i just started concerta and dr raised dose every week and now Im at max dose and its not doing anything" "concerta crash is too strong" etc.

DON'T SPLIT CONCERTA PILLS

Adhd medication dosage has an inverted U effectiveness curve aka larger dose isn't always better. Not to mention worse crash and side effects. (sadly doctors dont know this)

Upping the concerta dose should be a way slower process (wish I had known this)

You can lessen crash with and IR supplement or another smaller concerta dose some hours before the crash.

Most people need to take a tolerance break every weekend. Or a smaller dose on weekends if cold turkey is too tough.

Yes there can be severeish side effects the first week or 2.

Concerta isn't effective if you don't sleep/eat properly, you need also need to do some chores and exercise in the morning to really get some dopamine in the motivation tank.

Probably some info about how methylphenidate works and concerta release chart.

Just some general ideas.

Edit, some tips:

Don't try to do all of this at once btw. You might burn out from the amount of stuff.

This stuff was essential to me to get any real use out of concerta:
-Don't expect to "feel" something. Concerta is very subtle when it works.
-have meds and water bedside to take them as soon as you wake
-sleep at least 8H
-eat properly

-start building a morning routine once a routine is built its easy to do and an excelent source of motivation to carry you into harder tasks. Concerta should help with routine building and upkeep but adhd definitely still makes it hard. Expect failure, take a rest and try again.
-chores and physical activity in the morning easily build motivation/dopamine for the rest of the day
-start using productivity tricks they work so much better combined with concerta

-plan your day as soon as you wakeup or the evening before more detail/steps make it easier to follow
-I start a timer as soon as I take concerta to compare with this and to see how long things are taking.

-I have my phone set to make a notification every couple hours to remind me to stay productive
-sometimes if I procrastinate too intensly I set it to be an alarm instead
-some useful apps, can be any equivalent: mstodo, gcalendar, notion

-don't over do it, start slowly, prepare to burn out
-you can try med breaks on weekends to boost effectiveness but they aren't necessary for everyone check the post about tolerance.
-if break days feel awful consider taking a lower dose, personally 36mg on weekdays and 18mg on weekends and I don't get withdrawal sideeffects. Concerta tolerance usually lowers very quickly, one day to 2 weeks.
-You should probably use concerta daily and consistently at the same time everyday.

-You need good mental health: if youre depressed, anxious or what ever it will limit you so it's important to focus on first.
-Treat concerta as a booster, a tool. It barely does anything on its own.
-Correct dosage. Smaller dose is better to minimize side effects. 18mg is too little for most people but personally it was enough for me for a month. The max dose 72mg is a hard limit, there is basically no reason to go over it. A too high dose can worsen concertas effectiveness too.
Reducing dosage shouldn't be difficult either as concerta tolerance lowers quickly.

-Reward yourself for productivity.
-Make the barrier to start as easy as possible, step by step guide, snacks, good music. One that helped like crazy was buying waterproof gloves for washing dishes, just never realized how much the uncomfort made me avoid it.
-Physical activity!
-Taking meds with milk could reduce side effects

What do you think?
Did you already follow some of these?
I'd appriciate if ya'll could try some of these and see which ones help and how much. Feel free to leave your own tips too.

Tips/Tricks 🧠 Reminder: Mental Wellbeing

Hi folks. There's a lot going on these days across the world. Things can get overwhelming, and that's not factoring in other obligations such as school, work, or family.

So, this is your friendly mod reminder to prioritize your mental health and Wellbeing.

  • Quit the doomscrolling. Turn off the news and take a break from the extra anxiety

  • Prioritize good sleep (quality and quantity!)

  • Find ways to sneak in exercise. Aim for 150 minutes of moderate exercise weekly (or 75 minutes of intense weekly exercise), including 1-2 days with some kind of resistance or strength training. Something that you enjoy and find fun to do and keep up.

  • stay hydrated

  • Eat your colorful fruits and veggies. Minimize processed and fast foods.

  • Trim back on alcohol (especially if you have problems with sleep)

  • Never feel that you can't ask for help. Maybe it's just a chat that you need. Maybe it's counseling or therapy. Big or small, it's okay to get help. Showing that you are willing to seek help is one of the biggest signs of strength.

And as always, remember to practice kindness.

Dosage/ ℞ question 💊 36 mg not available can I do 18 mg x2?

So 36 mg is not available currently but the clinic mentioned that I could do 18mg x2. Anyone done this? Do you take them together or staggered approach? What has worked or not worked for you.

Side effects 🤕 Morning exhaustion

Okay I’ve been taking the 18mg ER for a little over a month now and have had very minimal side effects. I was given the opportunity to increase my dose but the 18 is working well for now. Anyways just the last few days I have been Exhausted when I wake up to my alarm. I am suddenly having a hard time falling sleep and I’m not doing anything different, either taking a sleeping pill or some melatonin (I did this even before this medicine). But I just cannot move in the morning. My alarm goes off I reach for my medicine and I physically can not do anything until my medicine kicks in just absolute exhaustion like can’t keep my eyes open, nodding back to sleep tiredness. That’s never happened to me before. I don’t get a crash throughout the day it’s pretty steady. Could this be my “crash” in my sleep early morning? Because it’s awful. Anyone else experience this? Other than that this medicine is fantastic.

Other question 🤔 Napping time :>

I wake up at 6am and take my Concerta then. When I get back home from uni at 3pm, I’m exhausted and need my one hour nap. But since taking Concerta, I haven’t been able to take my little nap, and it’s not like the exhaustion has disappeared. Now, I’m grumpy and tired for the rest of the day, so I thought y’all might have any tips.

Dosage/ ℞ question 💊 i am getting closer to the edge (pls read this)

Hi, I'm 18 years old and I'm going to confess everything that happened for the first time here. I would be constantly abusing Ritalin during the summer of 2025. Back then, there were times when I didn't sleep for three days, and sometimes I would stop taking the medication for five days so my body wouldn't get used to it. Three months later, when school started, my doctor prescribed 27 mg of Concerta, and I had no problems for eight months because I took my pills as directed. After that, I took another 3-month break because school was over. I started using 27 mg again two months ago because I didn't get into university, but my body has gotten used to it and now I finish my medication in 10 days. I'm afraid of becoming addicted, but even though I've taken excessive doses, I haven't shown any serious addiction symptoms. do you think i will become a addicted and what am i supposed to do

Dosage/ ℞ question 💊 If my dose is too much, does it not work as well?

Long story short I’ve slowly gone up to 54mg and I am hating it. 36 was amazing the first couple weeks but then didn’t seem like it was working as well so I went up… 54mg didn’t feel different at first but now I am not doing well— horrible appetite for one, but also hyperfocusing on unimportant tasks, distracted all the time, having trouble thinking of things, spacey… in some ways worse than when I’m unmediated. Not all ways, but some. So I am going back to 36…. But I’m really bummed that even 36 started out perfect and then things slowly got more difficult again. It’s still significantly better than my baseline, but the way I felt the first few weeks of 27-36mg was life changing. I can’t even explain how much better I felt in almost every aspect and I just want that back. Is this typical? Anyone get on too high of a dose and not feel anxious or jittery, but just like your ADHD symptoms weren’t well managed compared to a lower dose? I was only on 27mg for 5 days and I’m even wondering if 27 would be better. Or else I need to try a new med.

Side effects 🤕 Concerta seems to make my autistic traits more pronounced. Has anyone else experienced this?

Thumbnail
1 Upvotes

I wonder if anyone else has had a similar experience or has any advice?

I compared how I was doing when I wasn’t taking medication. Although Concerta helps me focus more throughout the day and blocks out distractions, it actually makes some things worse. For example, switching between tasks has become more difficult—I’m more “rigid” and can’t naturally move on to the next thing or stop what I’m doing. Crowds are also much more distracting. I can’t tune other people out as well and instead find myself involuntarily drawn to all kinds of little details. Even just five minutes in a crowd leaves me needing to go home and lie down, and I still don’t feel better after a whole day of rest. It’s awful.

I don’t know what to do. Sometimes I really do need it because of my ADHD, but when I take it, my autistic traits become more noticeable, and I can’t accept myself anymore! I don’t know how to describe it. When I’m not taking medication, I can let myself off the hook more and not force myself to do so much. But when I take it, I start forcing myself to meet those “normal standards” and act like a normal person.

I'm thinking about discussing a different medication or formulation with my doctor, possibly one that allows for smaller dose adjustments. I'm only taking 18 mg of Concerta, but the effects feel incredibly strong. It does make me calmer, but it also creates more problems.

Has anyone experienced something similar? I'd really like to hear how others have dealt with this, especially the difficulty switching tasks and the sensory overload.

Dosage/ ℞ question 💊 Concerta 36mg feels inconsistent, how do I know if it’s actually working?

I was initially on Ritalin IR, and I could definitely feel it working. It gave me this aggressive hit of focus, but I also felt pretty jittery, and the crash was a bit rough. I eventually switched to Concerta 36mg.
With Concerta, aside from extreme appetite loss and some insomnia, I haven’t experienced many major side effects. I take it on workdays and usually take a break over the weekend.
The thing is, lately, I genuinely can’t tell when it’s working and when it’s not.
I see people describing their experiences with stimulants, talking about feeling a significant difference in their thoughts, having a quieter mind, feeling more motivated, or noticing a clear before-and-after effect. Honestly, I don’t know if I experience those things to the same extent. I don’t feel particularly aware of the medication’s effects because the initial hit isn’t that intense. I’m literally always questioning whether my mind is racing or not… wait am i thinking about a couple of stuff at the same time again? and i notice that i sometimes do it.

I also can’t really tell when it starts wearing off or when I’m crashing. Sometimes I get tired in the middle of the day, but one cup of coffee is usually enough to get me up and running again. I don’t experience a particularly noticeable crash like I did with Ritalin IR, so I have no idea when the medication fades out.

For context, I’m diagnosed AuDHD, and I also have some minor OCD-like tendencies/issues, so I’m not sure if any of that affects how I experience the medication.

Another thing is that it hasn’t helped much with my executive dysfunction at home. I still struggle to get myself to do things, and I seem to focus much better when I’m working than when I’m at home.
Today, for example, I was sitting through a training session, and I was SO bored. I couldn’t concentrate at all. I kept looking around, wanting to get up and walk around, and generally feeling restless, even though I’d taken my medication.

Then, when it was time to actually work, I had some minor issues with distraction and maintaining focus, but nothing too severe.
I’m confused because I thought medication would help me regulate my attention better, but I still seem to struggle a lot depending on what I’m doing. I also don’t know whether I’m expecting too much from the medication or whether my current dose isn’t working as well as it should.

It helped a bit with my emotion regulation, not by far but kust a little. I can talk a bit calmer about things that usually triggered me. but if it got too much i’d explode

For those of you taking Concerta, how do you tell whether it’s working? Do you actually feel a noticeable difference in your thoughts, motivation, or ability to focus? Can you tell when it kicks in and wears off?
And for those who switched from Ritalin IR to Concerta, did you also find it much harder to tell whether the medication was working because the effects felt less obvious?
I’d especially love to hear from people who still struggle with executive dysfunction and boring tasks even while medicated. I’m trying to understand what a realistic response to Concerta is supposed to feel like.

Dosage/ ℞ question 💊 Tired and sleepy all day

Recently diagnosed with ADHD and prescribed Concerta 18mg. My first day on it felt really relaxing and peaceful now that my brain was quiet. I’ve been taking it daily and while initially I enjoyed the quietness, I also noticed I feel like it’s hard to think or even feel emotions. I also feel tired and sleepy all day that there I don’t have the energy or motivation to be productive.

Today was my follow up appointment with the psychiatrist and I explained feeling tired all the time, emotionally flat and lacking drive since starting the medication. I also noted these on the questionnaire prior to the appointment.

She said my scores showed an increase for depression (no energy, sleeping more, little interest in hobbies) and recommended I be put on an antidepressant. While I understand my responses can flag depression, I also feel like this is just a side effect of the medication. I asserted that while I’ve had bouts of depression in the past, I’m currently in a good headspace and I only started to feel this while starting the medication. She kept trying to encourage antidepressants.

I guess my question is… has anyone else dealt with intense sleepiness and tiredness? And does my psychiatrist’s approach sound dismissive or is it coming from a viewpoint that I can’t really see or understand?

Dosage/ ℞ question 💊 18mg jaw clenching and anxiety?

Long story short- I’ve been diagnosed with ADHD for several years now- initially I was prescribed 20mg Ritalin BID, but it made me feel like a crackhead- but mind you, I was drinking energy drinks and using an excessive amount of nicotine at that time. Got switched to adderall- Mydayis with a 10mg IR booster to Methylphenidate IR- 10mg TID, and found it way more calming and overall better than Adderall. I got tired of the crashes with IR methylphenidate, even though it was super effective; it would last 3hrs, then I’d crash. My provider recommended Concerta 36mg, but I opted to try the 18mg first. Today has felt so easy- I haven’t felt “stimulated” by any means, just everyday tasks I was able to do with ease. Today is day 1, and at like the 4hr mark, I noticed intense jaw tensing, and throughout the day I’ve had some jitteriness and anxiety and some brain fog. I’ve been busy running errands so I’ve basically had cokes today, zero water (until a couple hours ago,) and had a late lunch (took it around 10:30am because that’s what time I picked it up-ate breakfast at like 8am.)

ALL THAT TO SAY- would 36mg help and be more calming, or did I just do today completely wrong lol

Side effects 🤕 Short bursts of energy for 30 secs then back to being a brick?

I just sit there dissociated then from out of nowhere I feel my heart beat fast, my hair tingles and feel warmer on the inside. My eyes finally "clear up" and I can stare sharply at the present instead of past or future. I also feel like my brain popped a mint and overall creative and happy.

Then the pure horror happens, I return to drowned zombie baseline and feel even more "dramatic" and brain fogish than before but 30 mins later i'm back to just the usual amount of lifeless. This usually repeats a few times per day unless I take a 4 days break which gives me 2 good hours at the start of the day and then back to the shithole again.

Is this normal? The cockteasing mood swings aren't a new thing but they're way more noticeable on concerta.

Side effects 🤕 concerta anhedonia

hi,

i started concerta about a month ago, and my anhedonic symptoms have skyrocketed. i've plummeted into a weird, dissociative depression. i am trying wellbutrin, and i'm confused, because the concerta really helped at first. i don't have these problems with adderrall, but it's impossible to get where i live, and i can't afford a mail-order pharmacy.

i don't know what to do, as my ADHD meds help with functioning, but the depressive symptoms are killing me. i feel cold in my body and i've never felt so hopeless. what do i do?????

Dosage/ ℞ question 💊 Medication help/experiences

Hello all, just after some advice or peoples experiences please 😊 I started adhd medication about a 6 weeks ago. I started on concerta xl 18mg for 2 weeks then up to 36mg for 2 weeks and they had no effect at all. I then got put on 54 mg of concerta xl and am suppose to be going up to 72mg in a few days. However when I started the 54mg I started to feel this sort of euphoric feeling like a come up. Sort of how I felt when I was in my youth taking ecstasy (sorry best way I can describe it) just not as intense but a very similar feeling. I wouldn’t say my concentration got any better I was just sort of feeling wired. Is this how I should be feeling? I have no idea what I’m actually suppose to be feeling. I’m so confused and have no one to talk to about except my provider at titration meetings but that’s not for a week or so. Also some day I take the medication and after like an hour of taking I just crash instantly and feel awful for the rest of the day. Any help is much appreciated thanks everyone who takes the time to reply. I’m new to this sort of thing and not very good at explaining my self.

Other question 🤔 Concerta effect on emotional disregulation

I'm in my 50s, just got combined diagnosis. And started on Concerta XL 18mg on Saturday. I'm not sure if it's having much of an effect, but I'm currently working through titration, and trying to be patient/hopeful.

I'm wondering if anyone who has severe emotional disregulation (RSD/teary) has improved? I find the simplest things/setback make me want cry. I have read that Concerta can help, but I was hoping to hear from other sensitive bunnies.

Is there hope to feel more in control? I can't carry on this way, as it's really affecting my ability to function in the world. antidepressants never helped, and I'm hoping Concerta may help.

Articles/Information 🔎 trying to find a legitimate source of genuine Concerta 36 mg extended-release tablets

Hi everyone,

I'm trying to find a legitimate source of genuine Concerta 36 mg extended-release tablets for my personal medical treatment.

I'm based in Sri Lanka, and my psychiatrist has prescribed 36 mg once every morning. I also have an NMRA Personal User License authorising 90 doses.

I'd really appreciate some information:

  1. Which pharmacy or authorised pharmaceutical distributor in India supplied your Concerta 36 mg?
  2. Was it the original Janssen Concerta or another manufacturer's extended-release methylphenidate?
  3. Was it obtained through a regular pharmacy or a special-order/import process?
  4. Do you know whether the pharmacy can legally facilitate an international patient-specific supply, subject to the necessary permits?

I'm not looking to purchase medication from individuals. I'm trying to identify legitimate pharmacies or distributors and understand the proper legal process.

Any guidance or recommendations would be greatly appreciated. Thank you!

Dosage/ ℞ question 💊 Finding right dose?

For a little context, I am a 36-year-old guy who got diagnosed with ADHD-C about a month and a half ago. I started on Concerta 18 mg and now I’m up to 27 mg

I have noticed some positive changes in regards to impulsive behaviors. I’ve been able to quit coffee and don’t have any urges for alcohol or cannabis. I also notice in the morning I am more calm but feel a little bit spacey on Concerta.

On some days, usually in the afternoon, I can notice I can be more irritable and frustrated in the late afternoon with stressors. This usually occurs around 4 o’clock or so

Some coworkers of mine are diagnosed with ADHD and on Vyvanse. They say it has been life-changing for them and I’m not sure if I would consider my current treatment life-changing but definitely somewhat positive.

I am wondering if I should speak to my psychiatrist to inquire about Vyvanse. I know everybody is different. however, other people I’ve talked to have said that Concerta made them really angry or irritable and when they switched to Vyvanse, it felt right I’m confused because Concerta really doesn’t make me more irritable or angry.
