What causes cartridge heaters to burn out in the mold manifold?

We are experiencing a recurring problem with cartridge heaters in our injection mold, particularly in the manifold and nozzle areas.

The problem occurs only in certain zones. Even after repairing the affected zones or installing new cartridge heaters, the same problem happens again.
